It works for me and here is what I see in the logs when I middle click the bluetooth icon in the systray. I use KSystemLog to see.

i am taking a guess maybe see the shortcut maybe it’s assigned to something else
It seems to depend on the mouse. My Anywhere MX 3 can switch off BT with middle click
Sounds like your mouse’s middle-click button may be dying or dead.
