Certain keys are not working for me when combined with ctrl. These keys are, from my memory: R, G, D, H (I use colemak, so these keys may be different on a normal keyboard.)
xev is picking the keys up, they’re working to create and use shortcuts, but cat, and general programs are not.

It does sound like it could possibly be lower down in the software stack than where KDE Plasma sits - checking with your distribution forums could be helpful as well? There’s also generally a decreasing population of folks who are working with X11, and therefore running into X11-only issues, so that could be a factor as well.
