gpu problem when waking up from sleep

Hello, i installed kde neon some days ago, it is working fine but i am experiencing a weird problem that seems to be happening on any linux distro i tried installed so far

when i wake up my pc from sleep, it works just fine for some minutes and then, pc freezes but mouse pointer keeps working, then that freezes too, some seconds later it goes black and then image returns but is just random colors, it might go black again and repeat or stay black

i have to shut down pc and then it goes back to normal

it doesnt happen every time it sleeps, because i let it sleep many times per day, but can happen each couple days

searching on google it was suggested that it might be a gpu problem, gpu itself is failing, so i rant some tests and it passes just fine, unigine valley for linux just does fine for example, slow, but fine, this is a old gpu, a r7 250 2 gbs

not sure if it is a driver problem, or kernel or just the gpu itself dying in a weird way, it is a old gpu, but when it is working, it just does fine, youtube 4k at 30hz or 1080p 120hz
