I am using Kmail for my university’s Outlook email. At seemingly random intervals I get an error relating to authentication (see below image) and I will have to manually select Check Mail to update my inbox.
My university uses Duo for authentication. Is there a known solution or should I file a bug report?

I wonder, have you tried the Kontact flatpak? It has been pretty reliable in my experience (though I’m one of those people who never experience dealbreaking issues like that with KMail for whatever reason).
I think it is a bug. I have at the moment some IMAP accounts, an Outlook.com EWS resource, and a corporate EWS resource. I see the same intermittent errors as you but only on the corporate exchange account, which would be the same kind as your university.
