Windows open under

Programs, dialogs, and utility windows launched from within a program do not open on top. This is about 80% of all launches, but I can’t find what is determining this.

For instance I can launch Haruna by double clicking the file within Krusader and it opens on top, but if I launch it from within my media manager app (tinymediamanager), it always opens in the background. File choosers and other dialogs from most apps open in the background.

I’ve tried toying with focus stealing prevention on a per app basis with mixed results, but this can’t be the answer. Is something borked in my system?
